Join us for "Your Life. Your Choice.", a nurse-led workshop with VITAS Healthcare discussing advanced care planning. We welcome you to learn about advance directives, healthcare choices, and how to have meaningful conversations with your loved ones and healthcare providers. Sample planning forms will be available to take home.
